Incident investigation training in Kochi for Mumbai HSE teams

When something goes wrong at a Trombay plant, a Pir Pau berth or an offshore job mobilised from Mumbai, the HSE officer on duty is usually the person who has to write what happened and why. This half-day course teaches that job in a structured way, for ₹10,000, and it fits easily into one night in Kochi. It is an iMost certificate, not an OPITO course.

Mumbai's reports go to several readers at once

Mumbai's heavy industry is packed into a few belts. The Trombay and Mahul side of the eastern waterfront holds oil refineries, a fertiliser plant, a thermal power station and a terminal for storing hazardous chemicals, and Mumbai Port Authority runs the Jawahar Dweep oil terminal and the Pir Pau berths for chemicals and petroleum products. In such places a single event produces several documents: an internal report, a client or owner review, and a statutory notice to a regulator. The people who prepare them are rarely trained investigators, which is the gap this course fills.

In a Maharashtra factory, the occupier or manager reports an accident through Form 24 or 24-A on the online accident reporting system of the Directorate of Industrial Safety and Health, and the state factories rules also require a register of accidents and dangerous occurrences. The Occupational Safety, Health and Working Conditions Code came into force on 21 November 2025, and its sections 10 and 11 deal with notice of accidents and dangerous occurrences, with older rules continuing until new ones are notified. Your compliance team should confirm which reporting provisions apply to your unit today.

Oil and gas adds another layer. OISD lists incident investigation, to identify root causes and recommend corrective action, among its functions, publishes case studies and safety alerts, and oversees the offshore safety rules. Rule 11 of those rules requires an operator to give prompt notice of events such as loss of life, an explosion, a blowout or a major fire. Contractors mobilised from Mumbai to an offshore job feed their findings to the operator.

Trombay and Mahul industrial belt

Refineries, a fertiliser plant, a thermal power station and a hazardous-chemical storage terminal sit side by side on the eastern waterfront.

Mumbai Port Authority oil and chemical berths

Jawahar Dweep handles crude and petroleum products; Pir Pau handles chemicals, LPG and other liquids.

DISH accident reporting system

Maharashtra factories report accidents on Form 24 or 24-A through the Directorate's online system.

OISD role in investigations

Identifies root causes after incidents in the oil and gas industry and publishes case studies and safety alerts.

Mumbai roles that write or review investigations

HSE officers and safety officers at plants and terminals

They usually draft the first report and chair the review meeting, often within hours of an event.

Contractor supervisors at shutdowns and turnarounds

A contractor injury on a client's unit needs a clear account the client can accept, written in the client's format.

Shift in-charges and area engineers

They know the process and are asked to secure the scene, collect statements and keep logs and photographs intact.

Offshore contractor HSE advisers

Teams mobilised from Mumbai report to an operator, so near-miss and incident reports must stand on their own.

Where investigation sits among the other safety courses

Investigation training is the course you take after you have worked on prevention. If your problem is hazards that nobody has rated, start with risk assessment. If it is hot work or confined entry that is not controlled by permit, start with permit to work. Take incident investigation when the question is why an event happened despite the controls, how to prove it with evidence, and how to write actions that survive review. It is an iMost certificate and does not register you with any regulator. If your site expects a formal investigator qualification, check the client's wording before booking.

Risk Assessment Training →

Hazards are not yet rated, or the same event keeps recurring because nobody revisited the assessment.

Permit to Work PTW System Training →

The failure was in how permits were issued, received or closed out.

Basic H2S Training →

The event involved a gas release at a sour-gas facility; it is a separate half-day course.

The five modules, applied to a Mumbai site

The half day is interactive: classroom teaching, demonstrations, case studies and a group exercise. Assessment combines a written test, practical exercises and oral questioning.

  1. 01

    Introduction to incident investigation

Accidents, near misses and property damage, and the legal and regulatory frameworks. Bring your unit's own reporting forms and compare them with what you hear.

  2. 02

    The investigation process

Securing the scene, preserving evidence, and collecting data through interviews, photographs and reports. This matters most on a busy unit where the area has to return to service quickly.

  3. 03

    Root cause analysis

The 5 Whys and fishbone diagram, and how to separate contributing factors from systemic issues instead of stopping at an operator's mistake.

  4. 04

    Corrective actions and recommendations

Writing actions to SMART criteria and ranking them by risk, so that the actions list is something a plant manager can close.

  5. 05

    Case study and practical exercises

A group investigation and report presentation, useful practice for explaining findings to a client or a review panel.

For Mumbai plants, port-side operators and contractors

HSE managers who send several people on this course usually want a common method, so that reports from different shifts, contractors and sites look alike and can be compared.

Planning a group booking

  • Your HSE office can register at portal.imostacademy.in/register for team logins, contract pricing, group bookings, statements and certificate tracking.
  • We bill Indian companies in rupees with GST, take purchase orders, and extend credit only after an assessment.
  • Send a mix of roles, such as an HSE officer, a supervisor and an engineer, so the group exercise looks like a real review panel.

Mumbai to Kochi for a half-day course

Our campus stands at Njarakkal, on Vypin island. A Mumbai flight lands at Kochi after about two hours, and a taxi covers the rest.

BOM or NMI to Kochi

IndiGo, Air India and Akasa Air list direct BOM to COK flights; IndiGo and Akasa Air list NMI services. Schedules change, so confirm with the airline.

Cochin airport to the campus

Count on 40 km and about an hour by road. Arriving by train, you cross the Goshree bridges for the last 15 km from Ernakulam Junction.

Konkan Railway

A day or more each way, so it rarely suits a half-day course unless you add leave.

Fitting a half day around a Mumbai working week

  1. 1

    Choose the group

Decide who writes and who reviews. Send both, so the chair of the review panel knows what a good report looks like.

  2. 2

    Ask for dates

Ask our team on WhatsApp (+91 8075 787307) or at info@imostacademy.in which dates are available.

  3. 3

    Consider a second course

Basic H2S is half a day at ₹12,500 and hazardous materials is half to one day at ₹10,000. Ask whether either can be placed in the same stay.

  4. 4

    Travel and sit the course

Arrive the evening before. After the written test, practical work and oral questions, fly home that evening or the next morning.
